* Starting the repair work before getting block is an indication of poor regard to mandatory safety protocols. Very high traffic lead to saturation of tracks and getting block on such track for repair/maintenance become very difficult.
* The Rly staff get block for lesser than the required time leading to hurry and bypassing of safety protocols. The current accident appears to be a product of such situation.
* Underinvestment in IR over several decades have resulted in much lesser than the required capacity augmentation and the saturation of the tracks....
more... Introduction of more trains, increasing the permissible loading of freight train and increasing the speed of trains have made the tracks weaker and added to the woes/accidents.
