* Why IR always blow the LHB trumpet again and again as most important step to reduce accidents??? LHB coaches are not going to reduce the accidents, but can help reduce the number of casualties in event of an accident. IR have to work on multiple fronts to really reduce the accidents.
* IR itself admits that human errors is the reason behind most of the accidents happening on IR network. So concrete action is required in this area. Standard operating practices are there in IR which employees must be adhere to. But when these practices are not followed properly, safety is compromised and accidents happen.

Train operation is a very serious activity and there is no scope of any kind or error or negligence, the persons involved must understand this fact. Recruitment of well qualified and proper person and to train/ further educate them is essential to help discharge their duties.
Management of IR need to be very rigorous which reward the good job and strict punish the negligence of duty.
* IR must rely less & less on humans for safety, auto vigilance of machines is required to back up the human activity. For example Modernisation of signalling with warning in event of wrong action like jumping red signal.
* reducing the saturation of track by augmenting the track capacity. Over worked track put immense pressure on the working personnel which increase the probability of human error and accidents.

If Railways are open for suggestions, there will be numerous ideas which can help to address any kind of problem. Unless and until they change the babu culture, nothing is going to change.
