I personally feel the aggressive MRVC EMU and misreading of an adjacent signal due to line curvature is the major issue for such SPADs.
Signals should be installed properly, letting the LP/Motorman know which signal is for his train like for example at thakurli there r 3 signals left most of down Slow track, middle one for down fast track n right most for 5th Goods track. fast n goods track signals r confusing.
