I did some research on 1999 Gaisal Rail Disaster (I was 10 years old back then). This was due to quite a few conflicting reports on what actually happened. Now the accident may have happened at Gaisal, but I would say ASM's at PJP and KNE bear the major blame. And this is my understanding of the sequence of events

to the points between loop line and Up main line not being locked (it seems there was a diamond switch involved here), the Awadh Assam Express did not just join up mainline. But it actually cut right through it and joined the down line.

Here SP Chandra (ASM of KNE) made two further blunders:
a. He realised he had sent Awadh Assam Express on the wrong track, but informed only Panjipara and not Gaisal ASM. This was a huge mistake, as he assumed the Awadh Assam Express would move back to Up mainline at Gaisal not Panjipara (near 10 min run).

b. He did not follow-up with Panjipara ASM. And this would end up being the final nail in the coffin because what happens in point 2.

2. ~1.28 am, 2nd August 1999: The Awadh Assam Express arrives at PJP. Shockingly, it ends up having ~10 min halt here for Paper Line Clearance. The PJP staff only had information of the Awadh Assam Express. They did not bother checking with Gaisal staff.

3. ~1.38 am, 2nd August 1999: Awadh Assam Express leaves PJP with Paper Line Clearance (PLC) issued on the wrong Down mainline. The staff at Gaisal had already given Brahmaputra Mail all green signal to blast through Gaisal, since they had no knowledge of the wrong track issue with Awadh Assam Express.

4. 1.45 am, 2nd August 1999: Perhaps the worst rail disaster (official counts say 285, but this was due to gross revision in numbers). Back then government sources themselves said it was ~400.

Most of my analysis is based on findings of Ray Commission report.
