Yes both HOG ports and Guard van can be created in last coach itself.

I don't think EOG have step down transformer. It's not like railway using special EOG for HOG locos.
And why will normal EOG (not running with HOG loco) need step down transformer?...It's only task is generation.
I think Step down action is performed by HOG loco itself. Correct me if I am wrong
